Our primary care provider is Emily Fischer, MSN, APRN, FNP-C. Emily specializes in inclusive, trauma-informed care and has a strong background in women’s health, reproductive and hormonal health, adolescent medicine, and preventative medicine. She has multiple certifications and has completed education in postpartum nutrition, prenatal care, hormone replacement therapy, fertility, lactation, and labor support.

Primary care encompasses wellness, prevention, health maintenance, health coaching, education, diagnosis and treatment of acute and chronic illnesses.

Through our integrative approach, we consider all 8 dimensions of your wellness: physical, emotional, intellectual, social, occupational, financial, environmental, and spiritual. This gives your provider a clearer picture of who you are in the context of your life and the systems you are a part of.

Collaborative Care Management

Collaborative Care Management (CoCM) is a team-based approach to healthcare that involves a group of healthcare providers working together to deliver comprehensive and coordinated care to patients like you. Your team will include a Care Coach, Consulting Psych Provider, and Primary Care Provider.

A different approach to primary care

Our team-based approach flips the traditional care paradigm, from symptom-centered to patient-centered. This means that we provide continuous, wrap-around care rather than episodic care, building meaningful relationships with you, our patient.

Our integrative primary care provider works with a multidisciplinary team to provide you with various forms of wellness support and treatment options, each tailored to your unique needs.

We follow the principles of integrative medicine, blending conventional Western medicine practices with complementary techniques and mind-body medicine. Evidence-based practices are at the heart of all our treatment plans.

  • Referrals to local specialists as needed
